[GUIDE] Sample template or format for Affidavit of Loss in the Philippines

Have you ever experienced losing something valuable, such as a government-issued ID, an important certificate, or a key? Getting a duplicate of these things can be quite a hassle, which is why it is important to take extra precautions in securing things of value.

In the event of an actual loss, though, it is best to execute an affidavit of loss.

An affidavit of loss is executed by one who, by reason of accident or just plain inadvertence, loses a personal property, such as an ID, a credit card, or an important document, among other things. This affidavit is often required prior to the release of a replacement for the thing lost.

In the Philippines, a standard affidavit of loss typically adheres to the following format:

The above template may be used for any personal property apart from a motor vehicle. In the case of the loss of a motor vehicle, additional details pertaining to the particular vehicle lost must be indicated in the affidavit of loss. These details include the following:

  • Make
  • Motor Engine No.
  • Series
  • Serial/Chassis No.
  • Plate No.
  • Year and Model
  • MV File No.

Finally, note that an affidavit of loss needs to be notarized by a duly commissioned notary public for it to have any legal effect.
